Check your bottles for hairline cracks before filling. This happened about fifteen minutes after I filled from a keg. Fortunately I saw a bit of seepage and had put it in the sink.

I've had them do that while capping. Also has happened while prying off the cap...break a fricking bottle in half. I hate bottles.

I remember Doug Howe had a bottle cap come off a bottle so hard it flew up and cut his nose. Ahh the wonderful beers we used to run into during the HHHC.
